Includes preventative screenings, health education, medication reconciliation and refills, referrals if needed, lab orders (~$100 depending on what all labs are ordered) and a free lab-review follow-up appointment and healthcare planning.
Medication Refills (For Non-Established Patients) and Forms Completion $20

PCM/CCM Program
Principle Care Management (PCM)- ongoing care for one chronic condition
- ~$10-15/month for Medicare
- $20/month for Non-Medicare

Chronic Care Management (CCM)- ongoing care of two or more chronic conditions
- ~$10-15/month for Medicare
- $25/month for Non-Medicare
Minor Urgent Care (For Non-Established Patients) $35
Includes care for urinary tract infections, skin conditions, ear and eyes conditions, upper respiratory infections.
Mental Health Care $75 + Lab Fee (~$100) initial, $25 subsequent visits
